Overview:
In Week 7 you will take the final step in the nursing process, Evaluation. Post implementation of the health promotion plan, evaluation is necessary to determine success and where fixes or improvements need to be made. Since many health promotion projects are funded by local, state, and federal funds, there are requirements to show those funds are being used efficiently and effectively. Thus, outcomes need to be measured.
To evaluate, you must turn back to the plan and see if you have accomplished or moved toward achieving the objectives and goals you identified.
Even though you might still be working on your implementation task, you will need to begin the evaluation component of your project.
Your evaluation description will be the final part of your Health Promotion Paper. You will include both a conclusion to the evaluation section and you will add a conclusion to complete the process paper.
Objective:
For this task, you will:
Identify evaluation methods for your health promotion plan.
Add this element to your multiple part assignment
Complete with a conclusion and submit your Written Paper
Steps:
For the Evaluation Section
You must identify at least two items/activities/parts/characteristics of the Health Promotion intervention which should be evaluated. Here you are identifying what must be evaluated
Look at your Health Promotion Plan, you will want to evaluate whether you are achieving the objectives and the goal.
It is unlikely that there will be enough time in this course for you actually to evaluate the project; but you should be able to identify what should be evaluated if you did have enough time.
For each item identified, you will write a brief description of how those items would be measured/evaluated.
For each item identified, indicate who would be likely to do the evaluation ( for example: will it be the presenter at the time of client contact; will it be an auditor; will it be a quality control department; would it be the health promotion nurse themselves; or many other possibilities.)

Identifying and describing the above will likely require the use of some outside resources, for example, on the use of pre-test/post-test; or using surveys
Consider “participation” e.g., how many community members attend, sign up for, participate in, as a form of evaluation. Another evaluation metric to consider is surveying participants on how they view what they learned or experienced.
Do NOT use clinical evaluation methods. For our purposes, looking at decreases or changes in prevalence in heart disease, obesity, diabetes, smoking, etc., are not realistic metrics and not within your scope. Your evaluation metric should focus on engegment rather than long-term outcomes.
For the Compiled Paper
As you have been moving through the steps above you have been creating a document. This week, you will add your week 7 Evaluation, to Weeks 2 & 3 Assessment/Windshield Survey, Week 4 Analysis/Diagnosis, Week 5, Planning and Week 6, Implementation. As in all formal written papers assignments, your final paper should be in APA format and should include:
Title Page: Properly formatted with running head and page numbers.
Brief introduction reflecting the entire promotion:***Each section will also have its own introduction, but you will use the title page and running head that you created in Weeks 2 & 3. The heading for the week 7 section should be: “Evaluation.”
Body of the paper: This week the body of the compiled paper will include week 2 (assessment) week 3 (windshield survey) week 4,(analysis and diagnosis) week 5 (planning) and week 6 implementation. You will then complete and add your evaluation section, including an evaluation of each strategy you discussed in implementation.
Conclusion: You will include both a conclusion related to evaluation and a final conclusion to wrap up the Process Paper (written part )of the Health Promotion project.
Reference Pages:You will consolidate the references used in weeks 2 & 3, 4, 5, and 6, and add references from week 7. Each week’s references (except week 3) must must include at the minimum (but you will have many more):
