This course will focus on the integration of knowledge from current EBP, nursing theory, pharmacology and pathophysiology to inform student’s clinical judgment and ensure that offered care is holistic, effective and safe. The essence of applying a nursing procedure is to provide quality and safe care to patients while reflecting on cultural humility from childhood to old age.
The course will pay attention to interprofessional collaboration while coordinating care and managing diseases.
Summary Of Current Course Content Knowledge
The assessment will focus on the relevance of student’s class participation while meeting course goals and succeeding in current and future courses. It is through interaction with classmates and the tutor that one gets to explore the class materials, hence being in a better position to master course objectives. The paper will require students to review the objectives of each topic and answer the provided questions.

Class Introductions
This is the first assessment of the course that will require students to acquaint themselves with their classmates. In response, students will share their interests in the course and their potential challenges.

The assignment will also identify members of the interdisciplinary team used when the patient arrives in the ED. The paper will then discuss the roles assigned to the patient’s team members in each stage of the treatment. Lastly, the paper will discuss the appropriate discharge plan once the patient is stabilized in the ED.
Point to note: The discussion post submission should be 200 words, while the response to peers should range between 100 and 150 words.

Topic 3: Four Spheres Of Care In Complex Disease Management
Topic 3 DQ 1
The assessment should determine how racism appears in the healthcare profession. The paper should also identify internal and external system factors that lead to disparities in the healthcare sector. In addition, the assignment will focus on complex disease management whereby it will ascertain how healthcare personnel can identify and reduce discrimination and biases that affect patient care.
Topic 3 DQ 2
The assessment entails the organization of complex disease states that require care to be coordinated among multiple settings and providers over a long time. The paper should describe two payment and financial models in healthcare including quality of care, access, impact the delivery, fee-for-service, value-based care and capitation.

The paper will explain the roles of nurses in protecting people’s well-being. It will also identify strategies utilized by nurses to prevent the spread of transferrable illnesses during high-risk situations. The paper will also explain how nurses can protect themselves and their patients when there is a public health emergency.
Topic 4 DQ 2
The assignment will provide the difference between various healthcare surroundings across the continuum of care. More importantly, the paper will refer to the care of Timothy Smith in different environments while considering different transitions in his care.
The students should use the first letter of their name to choose between different environment options.
For instance, if a student’s first letter falls between A and E, they should discuss the difference between the intensive care unit and the medical-surgical unit.
Students between F and J should discuss the difference between medical medical-surgical unit and the inpatient rehabilitation unit.
Students between K and N should discuss the variance between inpatient rehabilitation units and outpatient rehabilitative services.
Students between O and R should focus on the difference between inpatient hospice care and outpatient hospice care.
Students between S and V should discuss the variance between ambulatory family medicine clinic and urgent care clinic.
The last cluster includes students falling between letters W and Z who will differentiate between a skilled nursing facility and home health care.
The paper should also discuss particular opportunities and challenges that one faces when navigating external and internal system procedures during care transitions and care coordination in the settings mentioned above.
Topic 5: Holistic Approaches In Complex Disease Management
